Changes in the second edition

The new edition contains updated data and explanations, new graphs, dozens of new photographs, and
color throughout.  Some of the more rigorous material from Chapter 2 was added to the appendix, and new
explanations for the various types of efficiency were added to the body of the chapter.

Also, the chapters have been reorganized for better flow.  The changes are as follows:
